What SBAR does well

SBAR gives a compact structure for communication. Situation describes the current concern or reason for the communication. Background adds relevant context from the source. Assessment contains assessment text already available to the user. Recommendation or Request captures the source request, question, or next step that needs review.

What a formatter should not do

A privacy-first SBAR formatter should not invent a recommendation, diagnose a patient, or decide what needs to happen. It should only normalize clearly labeled source text and keep unmatched notes visible for manual review. That makes it useful for handoff practice, student learning, and documentation cleanup without turning it into a clinical decision tool.
